1. String lights such as holiday lights, have a listing (label) of "Temporary (90 day max) installation and use only". If the lights are left up all school year for non-powered decoration use only, they must not be found plugged in at any time. If they are to be powered, they must not be installed for more than 90 days. Some of these lights also have a listing "For residential use only". These lights can never be powered in this building. (FYI: There are similar items such as many of the rope lights and LED string lights that do not have the manufacturers limitations that can be used with without limits.) See in guidance office room 160.
2. One power-cord can not plug into another power-cord. Each cord must plug directly
into a wall receptacle. See in room 114.

16. GFCI receptacle on the he front of the soccer field concession stand will not trip off.

Maintenance Dept. Issues:
1. Receptacle loose on top of workstation #4 in room 116.
2. Triangular emergency lights in hallway leading to cafeteria not working in test mode, in hallway in front of drama room.
3. A quad receptacle may be needed behind teachers desk in room 804.
4. Breakers # 36, and 38 tripped in panel DP in AG Building.
5. Exposed wiring beside HVAC unit in office beside office 623 that must be secured or removed.
School Staff Issues:
1. Electrical panels must not be blocked. A minimum working space of 30" wide and 36" in front of the panel must be maintained. This space extends from the floor up o 6' 8" above the floor. This area must remain clear and fully accessible for firefighter access. See room 310, coaches storage room beside room 623. Storage room beside band room.
2. One power-cord can not plug into another power-cord. Each cord must plug directly into a wall receptacle. See receptionist desk in guidance councilor area.
The following notes have been added to all School reports regardless if these issues were found at that school.
NOTE: Upon re-start of school, insure that all power cords that were left hanging during the floor cleaning process are fully supported and not hanging from wires.
NOTE: string lights such as holiday lights, have a listing (label) of "Temporary (90 day max) installation and use only". If the lights are left up all school year for non-powered decoration use only, they must not be found plugged in at any time. If they are to be powered, they must not be installed for more than 90 days. Some of these lights also have a listing "For residential use only". These lights can never be powered in this building.

Maintenance Dept. Issues:
1. Multiple Emergency lights in hallway leading to cafeteria not working.
2. Receptacle behind serving line in cafeteria looks in wall under sanitation placard.
3. Breaker #6 tripped in panel PL in main electrical room.
4. Cover on junction head on pump missing is off on AH-12 in outside mechanical room.
5. Breaker #18 tripped in panel DP outside of shop room 802.
6. Drop cords are not allowed to be used for condensate pump in room 804.
7. Outside receptacle on home team baseball dugout missing weatherproof cover.
8. Cord damaged on ice machine in concession stand for soccer field.
9. GFCI receptacle on scoreboard for baseball field will not reset.
10. Cord damaged on Pepsi machine and wall fan in concession stand for football field.
11. Junction boxes open in the outside mechanical room with AHU #3 south of gym. (Back wall up high)
12. There appears to have been multiple HVAC units that were changed out without the required permits or inspections. One of these units were noted on the previous inspection and a second one identified in this inspection at the AG building. See not #6 above.

Also, the red drop cord is missing the ground prong and must be repaired or replaced.

Maintenance Dept. Issues:
1. Emergency lights in hallway leading to cafeteria on BOTH SIDES of the boys restroom not working in test mode.
2. Emergency light in cafeteria on serving wall center not working. Can't test others due to height. Test all in this area.
3. Breaker in panel DB in outside electrical room is tripped.
4. Floor buffer in mechanical room 313 has missing ground prong.
5. Open electrical joints on stoplight sign in lab area between masonry and welding shops.
6. HVAC UNIT installed without permits or inspection on AG building. Several noticeable code violations also identified such as no disconnect, no GFCI receptacle, outside rating for duct and tape, etc.
7. Cooking Range in EC kitchen must be properly secured to prevent tipping.
8. Junction box around HVAC unit on mezzanine near band room has cover missing. Control wiring conduit also damaged in the same area.
9. Vacuum cleaner in room 211 missing ground prong.
10. Emergency generator annunciator panel in office indicates "Emergency Stop"
School Staff Issues:
1. Electrical panels must not be blocked. A minimum working space of 30" wide and 36" in front of the panel must be maintained. This space extends from the floor up o 6' 8" above the floor. This area must remain clear and fully accessible for firefighter access. See storage cabinet in room 310, boys locker room in gym, in gym office with uniform storage. Office/storage room 611, storage mechanical room beside the band room. Multiple notices on this issue near bandroom. A barrier may be needed to eliminate this issue.
2. Power cord has damaged cord in room 401.
3. Lamp in room 401 is listed for max 15 watt bulb but has a 60 watt bulb installed.
4. Large appliances like refrigerators are not be plugged into power-cords. Due to the heavier amperage load of these appliances, they must plug directly into a wall

receptacle. See office 615.

3. Flex conduit to pump in outside mechanical room 141 broken.
4. Panel PL in outside main electrical room has breaker #4 tripped.
School Staff Issues:
